PreParticipation Physical Medical Eligibility Form - Physicals are generally several pages, submit only the page titled "Medical Eligibility Form". It will have a box checked indicating either the participant is eligible to participate without restrictions, or with restrictions. TAKE A PICTURE OF THAT PAGE only and upload here. Adult Rowers: Skip this step. If you have any pertinent medical conditions the coaches should know about, email details to headcoach@rowgeorgia.com.

Youth Rowers: Required each season.

It's easy! Upload the specific page of the physical (detailed below) to this screen, and checkout ($0)! Your submission will be time and date stamped, and you'll be able to view it in your account history. This is the only way this form can be accepted, (no email or paper submissions accepted). Do not upload multiple pages. Instead, upload ONLY the page shown and described below.

Expiration:

This form is valid for 12 months from the date of issue.
If you have a physical that will remain valid through the end of the season, you can upload it.
If the physical is set to expire during the season, there will be a $25 admin fee to process a physical during the season, as it requires coach and volunteer follow-up and processing time. To avoid this fee, submit/obtain a current physical that will remain valid through the duration of the season.

What and How to Upload:

Which page? Within the physical, there is a page titled, "Medical Eligibility Form", and it's the only page of the physical required for registration, (do not submit multiple pages of the physical. Personal details and or HIPPA protected information may be on the other pages. The form must clearly display the participant's name, date, box checked indicating participation approval/disapproval or with details of restrictions, and the medical practitioner's signature. Example below.
Upload Instructions: Take a clear picture of only the completed page within the physical titled "Medical Eligibility Form" and upload it to this screen using the detailed instructions and images below, and complete "checkout" ($0). Important: If you have multiple youth rowers, initiate separate upload transactions under each respective name. Do not try to upload multiple rowers in the same upload.
Medical Provider Options: Can't get in with your pediatrician soon enough? Urgent Care same day appointments and mobile providers offer this standard sports physical and will provide the form.
