Registration Forms - Physical Preparticipation Medical Eligibility Form

MANDATORY EVERY SEASON for YOUTH ROWERS, regardless if new or returning rower. 

Georgia Sports Physical Form Instructions

Adult Rowers: Masters Rowers can skip this step. If you have any pertinent medical conditions the coaches should know about, detail those on the registration form where prompted.  

Youth Rowers:

1. Youth Rowers: Within your physical, there is one page titled, "Medical Eligibility Form", and is the only page of the physical required for registration, (do not submit any other pages of the physical). The form must clearly display the participant's name, date, box checked indicating participation, and medical practitioner's signature. Example above.

2. Upload Instructions: With your phone, take a clear picture of only the page within the physical titled "Medical Eligibility Form" and upload from this screen.

3. The physical is valid for 12 months from the date of issue. If your current physical is valid through 11/15/24 without changes in health status, you can use that one.

Submission Record: The upload will be directly integrated into your participant's Row Georgia account history. This is the only way the "Medical Eligibility Form" can be accepted, (no email or paper submissions accepted). Upload only the specific page of the physical via this screen. 

Important: The uploaded image must correspond to the listed participant and be associated with their profile. For additional youth rowers, please initiate separate upload transactions under their respective names.

Medical Provider Options: Besides pediatricians, urgent care and mobile providers also offer the sports physical.
